What Is Actually Happening in Your Brain When You Have ADHD
If you have ADHD, or suspect you might, you have probably heard a lot of explanations. Dopamine. Attention. Executive function. Focus challenges. All of that is real, but it rarely captures what it actually feels like from the inside.
From the inside, ADHD often feels like knowing exactly who you are and what you are capable of, and not being able to access it consistently. Like the gifts are genuinely there, and genuinely unavailable, sometimes in the same hour.
This piece is about what may actually be happening in the brain when that is the experience, why it varies so much from person to person, and what it might look like to work with it at a different level than most approaches reach.

ADHD is Not One Thing
One of the most important things to understand about ADHD is that it is not a single pattern. Two people with the same diagnosis can have completely different things happening in their brains, and an approach that helps one may be exactly wrong for the other.
What we tend to see in brain mapping work are three distinct profiles, each with a different mechanism and a different feel from the inside. The first is a brain with too much fast activity and not enough of the calming rhythms that create a brake. This brain moves quickly. Thoughts jump before the last one lands. Everything is coming in at once and filtering what matters takes constant effort. Think of driving at 150 miles an hour. The world goes by so fast that you cannot take anything in. You are tense, hyper-focused on what is immediately in front of you, and there is no room for nuance because the speed itself takes everything you have.
The second is a brain that does not have enough energy. Research going back to neurofeedback pioneer Joel Lubar identified excess theta brainwave activity in the frontal lobes as a common pattern in ADHD, particularly the inattentive type. Theta is the brainwave you produce when you are just waking up from a deep sleep. Kind of conscious, kind of not. Groggy. Dreamy. If someone asked you to read a complex document and synthesize it in that state, you would not be able to do it, not because of lack of intelligence but because the brain does not have enough energy for that level of processing. This is the person who is often labeled lazy or a procrastinator when the real issue is that they are pushing a car instead of driving it.
The third is both at once. Slow brainwaves underneath and fast ones on top. The brain is tired but it knows things need to get done, so it produces anxiety to compensate. The result is a system that procrastinates until enough pressure builds, then activates under stress and urgency, then crashes after. It works, but it is not sustainable and it is not comfortable. Knowing which pattern you are dealing with changes everything about the approach.

How Different Brain States Relate to ADHD

A state is simply how your brain is running in any given moment.
Sleepy is a state. Anxious is a state. Hyperfocused is a state. Calm is a state.
What you can access, how much energy you have, how clearly you think, and even how you feel about yourself depends heavily on which state you are in.
For many people with ADHD, moving between states smoothly is what may be missing. The brain gets stuck, whether in too much activation or not enough, and the flexibility to shift when the situation calls for it is not reliably available.
Dr. William Dodson has written about what he calls the interest-based nervous system, a framework that describes how many ADHD brains activate not based on importance or priority but based on interest, novelty, urgency, and emotional salience. When those conditions are present, the system comes fully online. When they are not, access drops significantly. This explains something that most people with ADHD know from experience but struggle to articulate. You can hyperfocus on something engaging for hours and be completely unable to start something important for ten minutes. It is not that the important thing does not matter. It is that importance alone may not be enough to activate your system.
Why It Affects How You Feel About Yourself
Here is something that does not get explained clearly enough about ADHD.
When the brain is in a good state, engaged and running well, self-concept tends to follow. You feel capable, clear, like yourself. The gifts that are genuinely yours are accessible and sometimes you are exceptional at things.
When the brain is not in that state, that can go the other way. Sometimes significantly. The same person who felt clear and capable an hour ago can feel like they are not good at anything, like they will never get it together, like something is fundamentally wrong with them.
That shift is not a true reflection of who they are. It is a state shift. The brain moved out of the conditions under which it functions well and the self-concept followed. When the brain re-engages, the feeling tends to lift. But in the low state it does not feel like a temporary condition. It feels like the truth. Understanding this as a neurological state change rather than a character failing is one of the most significant reframes available to someone with ADHD.
You are not unreliable. Your access to yourself is sometimes unreliable. Those are very different problems.
What It Costs To Function
One of the things that rarely gets acknowledged about ADHD is how much energy it takes to function in a world not designed for how your brain works. If you think nonlinearly, you are constantly translating. Your brain arrives at conclusions by jumping associatively from point A to point F. That part often feels effortless. Then you have to communicate it. You have to reconstruct the path, build B, C, D, and E in real time so someone else can follow the route your brain took in seconds. That reconstruction is work. It costs battery in a way the original thinking did not.
If your brain moves fast, you are spending energy filtering out everything competing for your attention. The conversation across the room. The texture of your clothing. The hum of a light. All of it is coming in and all of it has to be managed. And if you have learned to mask, to perform neurotypicality so effectively that nobody around you can see the cost, you are running an additional performance on top of everything else. Masking is not dishonesty. It is survival. But it is expensive. And because it works, nobody accounts for it. The expectations continue at a level the system cannot sustain without the performance. Most people with ADHD are working significantly harder than what is visible. The output may look the same as someone not working nearly as hard. The cost is not.
When It Might Not Be ADHD
Sometimes what presents as ADHD may be related to a sleep pattern. When the brain is not sleeping well, focus drops, energy drops, and emotional regulation becomes harder. It can look exactly like ADHD from the outside. In brain mapping work, this distinction is sometimes visible. When a sleep pattern is present, it changes what we are actually working with, and addressing the sleep issue may be the most direct path to the focus and regulation challenges the person came in for. This is one of the reasons a brain map matters more than a symptom list. Two people can describe identical experiences and have completely different things driving them.
What Brain-Based Support May Look Like

For most people, change is not dramatic. It is quieter than that. It looks like being able to start something without the internal battle that used to precede it. Following through more consistently without needing a deadline to activate. Less of the crash that used to follow periods of high output. More space between the low state and the identity collapse that used to accompany it. And over time, something harder to quantify but often described: more trust that the capable version of themselves is the real version. Not the exception they occasionally reach. The baseline they are returning to. Most people with ADHD already know their gifts. The work is about making those gifts more consistently accessible. That is a different goal than fixing something broken. And it tends to feel like a different kind of work.
